Abstract:

Background: The need for a rapid, noninvasive method for unwanted hair removal has led to the development of various Light sources for this purpose. Objective: To evaluate the efficacy of Intense Pulsed Light Source ((IPL)S) on unwanted hairs. Patients & Methods: In this open, uncontrolled clinical trial, 77 different anatomical areas in 34 referred patients to "Novin Didegan Clinic" in Tehran in 1380-1381, were treated by (IPL)S at fluencies ranging from 22 to 34 j/cm2 after their demographic data and the number of hairs were recorded. The 3 to 7 treatment sessions were accomplished 4 to 6 weeks apart. All of the patients were followed 6 months after the last Therapy. The quantitative data were  analyzed by paired t test. Results: Six months after the last Therapy, more than 50% reduction in number of hairs was noted in 80% of the treated areas; with mean reduction of 72±23 percent. Side effects were mild and reversible and occurred in ten patients. Conclusion: The (IPL)S appears to provide an effective and safe method for long term excess hair removal in various skin types.

Abstract:

Background: Photo-damaged skin is characterized not only by wrinkles and creases but also by epidermal and dermal atrophy, rough skin texture, irregular pigmentation, talengiectasis , laxity and enlarged pores. There is growing interest in the development of noninvasive methods to treat photo-damaged skin. Intense Pulsed Light ((IPL)), a non-coherent, broadband, Pulsed Light source is effective in the treatment of vascular and pigmented lesions of the skin. This study evaluates the role of (IPL) in the rejuvenation of photo-aged skin.Materials and methods: Full face skin of 20 women was treated for photo-aging using (IPL). Light emitted at the cut off filter of 560nm and energy emissions varying from 28-32 j/cm2 were utilized for 3 or 4 treatment sessions at intervals of 4 weeks. Patients and physicians evaluated rhythids, mottled pigmentation, and talengiectasis. Reviscometer & Mexameter devices were used to assess treatment results. Results: Two aspects of photo-damage including wrinkling and mottled pigmentation showed mild to moderate improvement in 25% and 20% of subjects respectively. Talengiectasis showed mild improvement in 5% of subjects. Twenty-five percent of subjects were satisfied with the overall result of their treatments. Conclusions: Non ablative skin remodeling using (IPL) is in its infancy. Results of this study showed that photo-rejuvenation by Intense Pulsed Light are not an effective method of skin rejuvenation.

Abstract:

Angioma serpiginosum (AS) is a nevoid capillary malformation developed congenitally or thereafter. The AS stops growing after puberty and may remain persistent throughout life. The Pulsed dye laser (PDL) and KTP are used to treat the AS usually after several sessions. We treated a patient with AS with only one session of Intense Pulsed Light ((IPL)) Therapy. A 40-year-old lady with Fitzpatrick skin type 3 and AS on the left maxillary and infraorbital area was scheduled to be treated with (IPL). The lesion was treated by a single pass of (IPL) Therapy with 8 joules/ cm2, in the telangiectatic mode. No anesthesia more than cooling was used, and a mild erythema was developed immediately after (IPL) Therapy, which subsided soon after the procedure. Two months later, telangiectatic lagoons were completely removed, and only very faint erythema remained. (IPL) is an effective and more tolerable device for the treatment of AS, with no purpura and discoloration, as frequently observed following PDL Therapy.

Abstract:

Background: Adequate control of acne is difficult, regardless of the various conventional modalities. Intense Pulsed Light ((IPL)) system is one of the emerging options that are become increasingly useful. Methods: To achieve the best (IPL) parameters we evaluated the efficacy and tolerability of (IPL) at 752-nm wavelength, 35 j/cm2 fluence, 55-ms pulse duration in comparison with 572-nm wavelength, 35 j/cm2 fluence, 101-ms pulse duration, in a 5 week, controlled, double-blind, split-face clinical trial. Final assessment was made by comparison of the changes in inflammatory and non-inflammatory acne lesions count and the Acne Global Severity Scale (AGSS) between two groups, based on standardized photography.Result: Fifteen female patients, with mean age of 23.53±2.47 years (range 20-28) completed the 5-week Therapy period. For both therapies, significant reductions (approximately 30%) in the comedone and inflammatory lesions count were observed (p=0.0024). There was no significant difference in the efficacy of the two treatments in reducing the percentage of comedone and inflammatory lesions count from baseline to 5th week (p=0.76 and p=0.61, respectively). Based on acne global severity scale (AGSS), no significant difference in the severity of acne lesions of the two treatments was observed at 5th-week visit (p=0.26).Conclusion: Considering the lack of significant difference between the two tratments and since greater risks are associated with lower pulse duration, the use of longer pulse durations is recommended, especially in darker skin phenotype. Further studies with larger number of patients are required to fully comparison of efficacy of these parameters in (IPL) systems for acne vulgaris.

Abstract:

Purpose: To assess the effect of a novel Intense Pulsed Light ((IPL)) Therapy on tear proteins and lipids in eyes with Meibomian gland dysfunction (MGD). Methods: Twenty‑ four eyes of 12 patients with MGD were recruited and received five overlapping flashes (565‑ 1400 nm) directed at the lower eyelid. The (IPL) parameters include intensity: 2. 5 to 6. 5 J/cm 2, voltage: 100 to 240 V, frequency: 50 to 60 Hz, input: 16 W, maximum optical energy: 23 J, pulse duration: <2. 0 ms, and repetition time: 1‑ 3. 5 s. Tear samples were evaluated immediately before and 2 weeks after (IPL) Therapy and included measurements of protein concentration, electrophoretic mobility by using sodium dodecyl sulfate‑ polyacrylamide gel electrophoresis, lipid profile assessments, and thin‑ layer chromatography (TLC) for phospholipids. Results: Significant improvements were observed in tear protein concentrations and molecular weight after (IPL) Therapy. The most pronounced effect was in the molecular weight of tear lysozyme, lactoferrin, and albumin. Tear lipids showed an improvement in the concentrations of total lipids, triglycerides, cholesterol, and phospholipids. On TLC, the tears in patients with MGD had significantly lower amounts of anionic phosphatidylethanolamine, phosphatidylinositol, and phosphatidylserine but amounts zwitterionic neutral phospholipid phosphatidylcholine were normal. These anionic phospholipids showed obvious recovery after (IPL) Therapy. Conclusion: (IPL) Therapy is effective in eyes with MGD. It improved tear protein and lipid content and composition. The anionic phospholipids were more responsive to (IPL) Therapy than were the other zwitterionic phospholipids.
